Step 1 - Create a Parent Profile

If you are the parent or legal guardian of a child who wants to participate in the Upward Soccer League at Triad Baptist Church, please complete the information form below to create a parent profile.

This step must be completed in order to register your child online. If you are not comfortable providing this information online, please register using the traditional paper brochure. Upward Soccer brochures can be requested via our online contact form, or may be picked up at the church office of Triad Baptist.
